How to Refer a Case
Choose the method that’s most convenient for you:
By phone
Call 609-259-8300 to book a consultation, schedule surgery, discuss a case, or obtain a cost estimate. If our doctors are unavailable, our Client Liaisons can provide immediate help (voicemail messages will be returned promptly).
- Referral app: Download this app to your smartphone, tablet, laptop, or desktop and use the form to refer cases from wherever you are
- Online: Submit your referral online using the form here on our website
- Fax: Print out our online referral form, complete and fax it to us at 609-259-8484
- Send the referral form with your client: Print out our online referral form, complete it, and give it to your client to bring to their first appointment. NOTE: Your client must call us at 609-259-8300 to schedule their consultation.
In an Emergency
Emergency cases don’t require an appointment or referral form. However, a call is always appreciated (if possible) to let us know a patient is on the way: (609) 259-8300
Preoperative Blood Work and X-rays
We recommend that you provide pre-op blood work and thoracic radiographs for all non-elective cases and for elective cases in patients >5 years of age. This information can be brought in by your client, mailed, or emailed to us using the referral form. Radiographs should be emailed to info@northstarvets.com.
